Minor Head Injuries and Nose Bleeds

First Aid for Bumps and Bruises

Head injuries

Children also frequently bang their heads and it is difficult to tell whether or not they have done any serious damage. Although, most head injuries are not serious and simply result in a bump or bruise. However, severe or repeated head injuries can cause damage to the brain.

Most blows to the head result in injury to the scalp only and although this is more frightening than life-threatening, it can still be a cause for concern. The head and face are very vascular which means these injuries bleed profusely and can be very scary!

It is very important to look out for anything unusual following a head injury; a severe bang on the head could cause swelling and damage to the brain and it is vitally important that you recognise any early and worrying signs of increased pressure on the brain.

What to look for

Head injury, first aid, first aid for life, help, advice, expert

Call 999 or 112 if a child:

  • has lost consciousness, even momentarily
  • won’t stop crying
  • complains of head and neck pain
  • isn’t walking normally

What to do

You may be able to treat minor head injuries from home.

If the child has not lost consciousness and is alert and behaving normally after the fall or blow:

  1. Apply a wrapped ice pack or instant cold pack to the injured area for 10 minutes.
  2. Observe your child carefully for the next 48 hours. If you notice any of the signs of brain injury (see below), phone an ambulance immediately.
  3. If the incident has occurred close to bedtime or naptime and your child falls asleep soon afterward, check in every few hours to look for twitching limbs or disturbances in colour or breathing. It is okay for your child to go to sleep – there is no need to keep a child awake after a head injury – but you do need to remain vigilant. If you are concerned in any way, wake them up to check on them properly.
  4. If a child can’t be woken or shows any symptoms of a brain injury (see below) call an ambulance immediately.

Suspected brain injury

Head Injuries

The brain is cushioned by cerebrospinal fluid, however a severe blow to the head may knock the brain into the side of the skull (see above) or tear blood vessels.

It can be difficult to determine the level of injury, so it’s always wise to discuss a head injury with your doctor. A clear indicator of a serious injury is when a child loses consciousness or has signs of confusion. These symptoms can come on at any time from immediately after the accident to a couple of days later. It is sensible to have your child sleep in the same room as you for a couple of nights following a head injury.

Signs of brain injury

Call an ambulance if a child shows any of these symptoms:

  • unconsciousness
  • abnormal breathing
  • obvious serious wound or suspected skull fracture
  • bleeding or clear fluid from the nose, ear or mouth
  • disturbance of speech or vision
  • pupils of unequal size
  • weakness or paralysis
  • dizziness
  • neck pain or stiffness
  • fitting
  • vomiting more than two or three times (it is not unusual for children to vomit immediately after an accident as a response to pain, so do not panic if your child is sick just once after a head injury).

If the child is unconscious

  • If they are breathing, roll them into the recovery position (on their side so their tongue falls forward in their mouth and any vomit can drain away), trying not to twist their neck or spine. Any head injury could have caused spinal damage.
  • If they are not breathing, start CPR.
  • Call an ambulance.

If the child is conscious but it is a serious injury

  1. Phone for an ambulance.
  2. Do your best to keep your child calm and still – try not to let them twist as if they have damaged their spine this could lead to a spinal cord injury.
  3. If there is any bleeding, grab a clean cloth and apply pressure.
  4. Do not attempt to clean the wound as it could make things worse.
  5. Do not apply forceful direct pressure to the wound if you suspect the skull is fractured.
  6. Never remove any embedded object from the wound.

NICE Guidelines:

The NICE guidelines suggest that if you are concerned about anyone for the following reasons you should ring 999 immediately.

  • Unconsciousness or lack of full consciousness (for example, problems keeping eyes open). difficulties with understanding, speaking, reading or writing; decreased sensation; loss of balance; general weakness; visual changes; abnormal reflexes; and problems walking. since the injury. Any suspicion of a skull fracture or penetrating head injury.
  • Any seizure
  • A high-energy head injury resulting from a forceful injury.
  • Get checked out by a health professional for anyone who has experienced any of the following:
  • Any loss of consciousness (‘knocked out’) as a result of the injury, from which the person has now recovered.
  • Amnesia for events before or after the injury (‘problems with memory’).
  • Persistent headache since the injury.
  • If there have been any vomiting episodes since the injury.
  • If the victim has suffered a previous brain surgery.
  • The patient has a history of bleeding or clotting disorders.
  • Current drug or alcohol intoxication.
  • There are any safeguarding concerns (for example, possible non-accidental injury or a vulnerable person is affected).
  • Irritability or altered behaviour (‘easily distracted’, ‘not themselves’, ‘no concentration’, ‘no interest in things around them’), particularly in infants and children aged under 5 years.

Nose bleeds

Children often have nose bleeds. They can be a result of warm weather or exercise, which dilate the small blood vessels in their nose, or they can be the result of picking or poking their noses, or running into things. As a result, it is really important to keep an eye out for nose bleeds if the weather is warm or if you have been fasting.

If a child has a nose bleed:

  • Sit them down.
  • Grab something absorbent to catch the blood.
  • Lean them forward, pinching the soft part of the nose, just above the nostrils. Leaning the child forward whilst applying pressure to the nose will allow you to see when the bleeding has stopped and will avoid the blood trickling down the back of their throat which could make them sick. You should apply pressure and try to compress the leaking blood vessel against the inside of the nose to stop it bleeding.
  • Keep changing your grip until you have got to a point where no blood is coming out.
  • Keep applying pressure for at least 10 minutes.
  • Release pressure slightly and if it starts to bleed again hold for another 10 minutes.

If it really won’t stop bleeding you will need medical help. Advise them not to pick, poke or blow their nose. If it starts again you will have to apply pressure once more.

Special Situation

If the nose bleed has been caused by trauma, or a punch in the face, controlling the bleeding may be difficult but you need to try as loss of blood is dangerous. You should apply a wrapped ice pack, keep applying pressure and get medical help.

My mom got mad and hit me over the head with a big cast iron skillet. I was a teen. Now, I’m old. She got mad because I had some crumbs on a plate. I had put the plate in the sink. After she hit me, my nose bled, but I guess it was nothing serious.

Dear Lee, I am sorry to hear about your head injury. Clearly being hit over the head with something that heavy can lead to a major head injury and your nose bleed could have been a sign that you were suffering from serious damage and possibly even a skull fracture. You story illustrates clearly how injuries should always been seen in context and not in isolation. Thank you

Dear Lovely, If you are worried about your niece and she is showing other worrying signs following a head injury, in addition to a nose bleed, please get her seen by a medical professional. If she only has a nose bleed and it was a very minor head injury and there are no other worrying signs, then keep a close eye on her and get her checked out if there is anything worrying or unusual.

            Dear Emma,
So sorry to bother but I don’t know what else to do. My 2 year old daughter had strep throat 10 days ago now and she just finished her antibiotic. However now a phlegmy cough has developed since day 7. It is now day 10 and she has fallen face first and hit her nose pretty hard and got a small bruise on her forehead. Immediately after this hit, her nose started bleeding heavily. Cleaned her up. This was 5pm. Since then, every time she has that cough come back, which is coming back primarily at night time and eary mornings, she is now bleeding from her nose as a result. What can be the problem??
I’m sorry to bother with this but I have no clue what to think or do but just take her to the doctor in the morning. But want an idea of what this might be. Anything you could share would greatly appreciate. Thank you.

          • emma

So sorry to hear about your daughter. Please take her to the doctor so she can be properly examined. I am afraid it is impossible for me to give any suggestions remotely. It doesn’t sound like there is anything hugely worrying going on, but please get her properly examined. I do hope she is better soon. Best wishes Emma

My son(7months) hit him head after falling from a bed. Hear nose bleeds rightaway. I’m worried. What should we do?

          • Emma Hammett

My nephew was standing behind the door and I accidentally slammed the door open and he was behind it so the door hit his face and his nose started bleeding. I only bled for about 3 to 5 minutes but then it stopped but there was a lot of flooding should I get medical health or is it something I should be concerned about?

          • Emma Hammett

Dear Stella, if your nephew seems confused or under the weather in any way or exhibiting signs of shock I would absolutely get him checked out.

          • Julie

My son was pushed down stairs at school 3 days ago, he hasn’t been himself since. Very in a shell. Looks confused and he’s complained of headaches and now is having random nose bleeds.

          • Emma Hammett

Please get him checked out, as you are obviously very worried

          • Donella

My son had a nose bleed at 5 this morning. Then he sneezed maybe 30 mins ago nd blood came out. What’s going on

          • Emma Hammett

Children often have nosebleeds as they have tiny blood vessels in their noses that can expand and burst when they get warm. This often happens in bed. If he subsequently sneezed, and the vessels were weakened then this would explain what happened. However, if you are worried at all, please seek further medical help. Best wishes Emma

I hit my head while wrestling 2 days ago and have been told I have a concussion. Today I sneezed and blood flew everywhere. It wasn’t bleeding before and wasn’t bleeding after. It’s just spewed blood when I sneezed. Is that normal or should I get checked out.

Hi I’m not sure if you can help me. But my Daughter had an accident where her and her friend we’re playing tig and her tooth bashed into my daughters head and cut her. She’s fine it bled loads this was yesterday. But its still bleeding a little bit is that normal?

          Reply
          • Emma Hammett

Please don’t worry about your daughter’s scalp bleeding as it is extremely vascular to give a great blood supply. Consequently it may still bleed occasionally following the injury. Keep an eye on the wound and your daughter and if you are worried, get it checked out.

          • Ewet

Please I have a question my 2 years old hit his head with a little wound on it with just small amount of blood flow but after a day or two he started bleeding from his nose e everyday for about 4 to 10 minutes what could be the cause

Hi Emma, today I was playing football with friends and slammed my head on a metal pole. I went to the emergency room and they glued it together and confirmed i did not have a concussion. However, as I am writing this at home, one nostril in my nose is continuously bleeding. I usually do not have nose bleeds, is this something to be concerned about?

          • Emma Hammett

Dear Asif, If she bumps her nose when she falls that would explain the nosebleed. Otherwise it may just be a coincidence. If it happens again and you are worried, please get them checked out. Best wishes Emma

          Dear Emma,
My two year old son tripped and hurt his nose. There was very little bleeding, more like bloody boogers came out. He stopped crying after a couple of minutes. I don’t see any bruising or swelling and he is back to his normal self. He is very clumsy unfortunately and he has minor bumps and falls every other day. Should I take him to the hospital? I’m worried because he bumped his head on the chair yesterday evening and now he hurt his nose.

    • emma

Dear Fiza, I am so sorry you 9 year old has had these problems. It is highly unlikely that the sickness, temperature and nose bleed were related to the head injury as head injuries do not generally cause a raise temperature and we would expect the vomiting to occur immediately or not long after the head injury if they were as a result of it. The same goes for the nose bleed, 4 days after is unlikely to be related to the initial injury, unless they had bumped their head oce more.

I fell and hit my face on my wall, my nose bled lightly directly after the fall for around 30 seconds. The following day I had uncontrollable snot dripping out of my nose for about 5 minutes. Then about two days later my nose bled again. Is there a chance my nose is broken?

Hi Emma, i banged my head on the wall and after say 5 minutes later, nose bleed. Also swelling around my right eye and right eye looks smaller than the left one. Since then it never bleed heavily. However I do see some clots of blood of dark red color when i cough since last 2-3 days. I don’t have a headache and no omitting since after head injury.
Before head banged, i am suffering through cough and cold and mucus is coming out through lungs.
Is there any possibility of skull fracture or brain vein damage or rupture inside the brain ?
I also would like to ask you – Where does blood go if someone has internal brain bleeding ?

Dear Manish, so sorry to hear of your accident. It sounds like you may want to get yourself checked out. I think it highly unlikely to have a skull fracture or any brain damage from your injury. When someone has a brain haemorrhage the blood goes into cavities surrounding the brain and cause a build up of pressure. I hope this is helpful. Emma

When he would rub it he would scream for about 30 minutes. We called dr & she said to just give Motrin and cold compress since his nose isn’t crooked. Is this the same advice you would have given?

Dear Kayla, Glad he is okay. Not sure why he would have a nose bleed as a result of a minor fall, unless it was a coincidence. Did he cry a lot when he hit his head as sometimes that can increase pressure and lead to tiny blood vessels in the nose bleeding slightly.

Hi emma, the daughter of our neighbor 15 years old had a frequent nose bleed without any reasons, we are worried about her coz when she was 5years old she had a serious head injury. Is it possible nosebleed is due to that incident before?
What should we do to help her?

    Reply
    • emma

Dear Arlene, Thank you for your message. It is extremely unlikely that these nose bleeds have any link whatsoever to a head injury 10 years ago. Far more likely to be small blood vessels in the nose rupturing when she is warm, or has been doing exercise. If there is a monthly pattern to them, it is possible that it might be linked to her monthly cycle and a form of endometriosis – if they are worrying, please ask her to see her doctor. It is possible to quarterize the nose and seal the little blood vessels causing the problem, this is a simple and relatively painless procedure.

A day and a half later he had a small bleed in one nostril and then the other, a lighter color but still blood, could this be related? Do nosebleeds ever happen more than 24 hours after a head injury?

Saturday we noticed a slight left black eye around the inner eye/nose area, then Sunday the other eye same area. Tuesday, a week later, he had a small nose bleed, now this morning he woke up from a major nose bleed… When it 1st happened he had a headache before the goose egg appeared, but hasn’t complained about anything since & acts normal. I’m concerned over the nose bleeds, unsure if anything could be going on that we can’t see.

Dear Genie, I am so sorry to hear about your son and dog and hope they are both okay now. It is unlikely that a nose bleed a week after a head injury is directly related to the initial impact of the head injury. It is more likely that he became warm for some reason and the tiny blood vessels in his nose dilated and burst, causing the nose bleed. If he shows any unusual signs and symptoms, or you are worried in anyway at all, please get him checked out. Best wishes Emma

Dear Lorraine, Sorry to hear about your son’t accident. It is highly unlikely that this most recent nose bleed is directly related to his head injury. Dear Lorena, I am so sorry to hear about your accident and do hope your son is okay now. It is unlikely that his nosebleeds are related to the accident if they are still occurring 2 weeks afterwards. Often small children get nosebleeds when they get warm, as the tiny blood vessels in their noses dilate and burst. It isn’t usually a major concern and most times they grow out of it. However occasionally, if the nose bleeds are extremely frequent and causing distress, doctors will do a minor procedure to stop them happening. They would not consider this until your son is a lot older.

Dear Valerie, so sorry to hear about your son’s accident and I do hope he is fine now. I ran into the back of someone’s head about 7 hours ago and I felt dazed after the impact and began to bleed from my nose. It was on and off for about 30mins after that till it dried up completely in my nostrils. I have still been experiencing pain on on my forehead since then and near my eyes. How serious is this and what do you suggest I do please? I am a 17 year old boy.

My son is 2 years old he jumped off the couch and fell straight to his head. It was very obvious that he went to his head first and his feet followed almost coming back to his head.. Such a weird thing since it was in slow-mo.. Anyways he cried for a minute and then an hour later he had a mild nose bleed and saying his mouth hurts? Checked his pupils, they are dilating at the same time and the same size. Any recommendations? Just freaked me out small bump where most of the impact was. Btw I’m not sure if it makes a difference but we have hardwood floors but where he hit we have a rug with semi-thick carpet padding under it.
    Thanks in advance!

It sounds like it may be swelling that has caused your son’s nose to change shape. Applying a wrapped ice pack may reduce this, however I would strongly suggest you get him checked out by a health professional.

Hi my 15 month son has blood coming from one nostril. Not a lot just enough to dry around his nose, it dark brown rather then red. He has not had any incident as in a fall or bump today. At the time he had only a nappy on as he had got covered in his breakfast so was not hot. I’m not sure what could have caused it. Should I get him checked? Thank you

My 1 yr old fell down stairs he was bleeding from nose…it was swollen and bruised and very sore. I applied ice it stopped bleeding after 10 min. He had a nice boo boo on nose and chin. His nose is healing its been a week. But now he has a fever and is vomiting..loss of appetite. Can this all be associated with his falling down stairs? Or is this probably just a cold and I’m thinking to much into it?

My son is 2 years and a half he has been having constant nose bleeds since he was 10 months old. And he bleed about once a week sometimes twice a week. Every time I take him to the doctors they say it’s cause of the weather. But I don’t know what else to do. He just had a nose bleed about 20 min ago. It only lasted a 3 min. Sometimes it seems like they are blood clots. He also has had accidents since he was 10 months. Like 5 accidents.

Dear Katerin, many children have nose bleeds as they have tiny blood vessels in their noses that rupture when they dilate, when the child gets warm. It is possible to get your son’s nose quarterised to stop this, but most times they grow out of it as the blood vessels become more robust. Best wishes and good luck Emma

Hello. It is great seeing the answers being kindly made and just reading them helps me much already.

Earlier today, my son fell from my shoulder onto the concrete ground as he gets playful and swung his body on me. He flipped front and fell on his back(I guess) in front of me and his nose is bleeding. The amount is not much and he has records of nosebleeding during his sleep time to time.
But he seems exhausted since the fall. Now, after about 1 hour, he seems okay but should I consider seeing a doctor?

I am very glad your son has been checked out at the hospital. Children have small blood vessels in their noses that often dilate and rupture when they get warm. If it continues to be a problem ask your GP to refer for further investigations and possible cauterisation. Usually they grow out of this problem though

Hi emma, my son is 20 months old and a month ago he hit the top of his nose which caused a cut. The swelling was instant and very bad so I took him to a&e they said it should go down after time but there’s a really hard lump there still and he tells me it’s sore. I tried to go to the doctors but she said I was silly for thinking it could be anything bad with it only being a month. But do you think it could be anything else cos it’s still very noticeable and when people see him for the first time they instantly ask how he bumped his nose.

    Reply
    • Emma Hammett

Dear Nicolle, It is highly unlikely that the swelling relates to the bump over a month ago, but if you are really concerned ask your doctor for a referral to a hospital specialist.

Um… hi Emma I’m 13 years old and I took bath with too hot/warm water it made me very dizzy I tried to walk back to my room as quickly as possible to lie down but fell on my face on the wooden floor near the door of my room this was two days ago I have a hematoma on my forehead it seems to be recovering and going down, even though it’s probably not serious, I’m still worried will I have to go to hospital? Will I need surgery?, After the fall I’ve had tiredness and take more naps is this a cause for concern?,though it may be because before the fall I usually slept quite late and got up early but now I sleep earlier, as far as I know when I fell I didn’t black out because I remember my mom calling me and asking if I fell I think I also remember hitting the floor, I only have a slight uncomfortableness in my neck, but it’s probably due to the fact I usually sleep on my side and not my back, my nose did bleed a bit when I first fell but it was probably because I bumped my nose on the floor it completely stopped bleeding a few minutes afterwards,

I’m still very worried and hope for a response
        Sincerely, Molly

Ten minutes ago, I just experienced a slight clogging of my other ear, followed by a slight ringing, which is now gone, but my nose was bleeding a tiny bit (not much at all) on the side of my head where my ear was just ringing. Should I be worried? Thanks so much!

My father once punched me so hard I flew backwards into the kitchen cabinets and had the wind knocked out of me. Since I was only five years old, I lost my breath and my nose wouldn’t stop bleeding for hours. After that I had nosebleeds every other day to the point I had to get my nose cauterized at least twice a year. Once I filled a bath towel up until it was soaked with blood. As I got older the bleeding wasn’t as often or as bad, but during times of stress my nose would bleed profusely. Even now that I’m in my 50’s, whenever I’m stressed my nose bleeds. Is this from that incident at 5 years old?
